DNR Forms

The office of Emergency Medical Services (OEMS) is now distributing the new Durable Do Not Resuscitate Form. In order to get these new forms, you must fill out the DNR Request Form and send it to the OEMS. Please note that you must get this new form to comply with the new Durable DNR laws. Under the new laws, Durable DNRs do not need to be renewed yearly, rather they are in place for as long as necessary or until revoked. Click here to download a printable form.
